After Hours Clinical Services Relocation

EFFECTIVE 15 APRIL 2011

On Friday, 15 April 2011, our After-Hours Clinic (run by Lourdes Family Practice) will move into a new building on the Deborah Hospital campus. The new location is 212 Trenton Road, Browns Mills, NJ, on the second floor of the Cymrot Building. The Cymrot Building is adjacent to the main Deborah Hospital and is accessed using the same main hospital driveway (building is immediately on the left after turning into the hospital parking lot). Lourdes staff will also be positioned in the current clinic location to re-direct our patients to the new building if necessary.

This new location offers dedicated space for our After-Hours Clinic and allows Lourdes to expand hours of operations. Beginning 15 April 2011, the expanded operating hours for the After-Hours Clinic will be Monday-Friday 1630-2200; Saturday 0830-1330; and Sunday & Holidays* 1100-1600. The new phone number for the After Hours Clinic will be (609) 735-2030.

Our 87th Medical Group leadership has toured the new site with their counterparts from Lourdes to ensure the quality, accessibility, and patient-centeredness of services offered through the After-Hours Clinic. If you have any questions, please contact the medical group at (609) 754-9306 and ask to speak to a member of the leadership team.

* Please be advised that holiday hours at Lourdes (1100-1600) will be in effect on Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day and New Year's Day. For all other Federal holidays (e.g., Martin Luther King Day, President's Day, Columbus Day, Veteran's Day) regular daily clinic hours (1630-2200) will be in effect.

For TRICARE Prime beneficiaries enrolled to the 87th Medical Group, no preauthorization will be required to go to the After Hours Clinic at Deborah and for active duty and their family members enrolled in TRICARE Prime; there will be no copayment for these services. For Prime-enrolled retirees and their family members, a $12 copayment will apply. As always, patients should proceed immediately (no referral needed) to the nearest Emergency Room for life-, limb- or eyesight-threatening conditions.
The 87th Medical Group strives to take care of your family's medical needs. The new Air Force Family Health Initiative is an active, patient-centered approach to establishing a "medical home" for each individual enrolled to the medical group. Your primary care team consists of doctors, physician assistants, nurse practitioners, nurses and medical technicians.

"My commitment to you is to provide exceptional care for you and your family within your medical facility and ensure your needs are met, no matter the time of day or night," said Colonel Burks, 87th Medical Group Commander. "Our goal is to ensure patients will be cared for by their family health team whenever they visit the medical group for primary care."

AudioNotes

The 87th Medical Group is now offering an automated messaging service to provide you with laboratory and radiological results in a timely and reliable fashion. You will have access to a personal secure voice mailbox that your provider will use to leave important messages. The system offers you the comfort and convenience of:

- Accessing your normal lab and x-ray results and obtaining any additional instructions from your provider
- the ability to check your voice mailbox 24 hours a day, 7 days a week
- an automated phone call reminding you of un-played messages in your personal voice mailbox so you can call-in and listen to your messages if you have forgotten

How Does the System Work? You must receive a telephone message from AudioNOTES indicating you have results waiting to be reviewed. Only then you will be able to dial and access the system.
You will be asked to enter your sponsor's social security number and your Family Member Prefix (FMP) number (i.e., 30, 01, 02, etc.). You will then be asked to enter your eight-digit date of birth (mmddyyyy). Next, you will be asked to create a Personal Identification Number (PIN). Once your PIN has been established, you will use that number every time you access the AudioNOTES system. Your PIN will be valid for 3 years. You and your medical team will be the only people that know your PIN. We request that you do not share your PIN and keep it in a secure location.

You always have the right to speak to a member of your medical team. After listening to your message, if you have any questions, please call the clinic at 1-888-DRS-APPT and leave a message for your provider stating you would like to discuss the message left in your AudioNOTES mailbox.
If you lose or forget your PIN, don't worry. Simply call AudioNOTES Customer Service at 609-754-9232 during normal duty hours and our staff will assist you in resetting your PIN. You will then be able to create a new PIN the next time you access AudioNOTES.

This messaging system will allow you to save messages and listen to them up to 7 days later. Messages that have not been accessed will be saved for 60 days before they are erased. If you have any questions about your messages after they have been erased, please contact your medical team.
Instructional pamphlets on how to access and use the AudioNotes system will be available at the 87th Medical Group.

We hope that you will find the AudioNOTES messaging system convenient and easy to use.
